AgeOptions provides two Take Charge of Your Diabetes program options. Choose the program that is best for you.

Take Charge of Your Diabetes Basic – A seven-week workshop consisting of weekly two-and-a-half hour sessions. Each class is led by two certified facilitators.

Take Charge of Your Diabetes Plus – An eight-week workshop for participants who are referred by a doctor and have Medicare or other insurance. This program is accredited by the American Association of Diabetes Educators. Classes are held weekly for two-and-a-half hours per week. This workshop also includes two individual assessments conducted by an AgeOptions registered dietitian. During the initial assessment, our dietitian assists participants in identifying educational goals to better manage their diabetes. A follow-up assessment in the eighth week looks at future needs and includes developing ongoing plans for self management.
